To locate participating providers in the 2008-2009 Concordia Theological Seminary Student Health Plan, please go to www.uhc.com, and click on “Find a Physician” in the top right corner of the home page.  Then click “Find a Physician” again on the next page, this will bring up a form needed to complete for location, specialty, etc.  Please note that on this page, the first question after “Location” is “Which health plan are you interested in?”  Please select “United Healthcare Options PPO” from the drop down menu, (it is the first available option).  This will get you to the appropriate list of medical providers, and from here you can find a great deal of information about available medical providers.
