HEALTH INSURANCE REQUIREMENTS
Student and Family Medical Requirements

The Requirement for Students:
CTS requires that all seminary students taking 7 or more credit hours at CTS be enrolled and billed for the CTS Student Heath Plan (for Group coverage). A student may not insure himself through an individually underwritten plan. Waiver Petition requests will be considered only for students who have valid coverage through the Military or VA Plan, or a group plan through one’s spouse who is employed, or through a parent’s group plan. Waiver Petition Forms will be available on-line after August 15, 2008. The fall waiver petition deadline is September 15. Go to www.uhcsr.com to find the waiver form.

Insuring spouse and children:
While the student plan is required for all students, the CTS dependent plan is recommended for spouse and children. All students are encouraged to insure their dependent spouse and children (up to age 19) through the seminary’s group plan for dependents. Please note, however, that dependents are eligible for the Seminary’s dependent plan only if the student is enrolled in Seminary’s student plan. Both the student plan and dependent plans can be reviewed at www.studentresources.com. If you have any questions please contact either The Harbour Group directly at (800) 252-8160 or the Dean of Students Office (260) 452-2152 ASAP.

Vicar Requirements And Information

CTS requires that all seminary students (which includes all vicars) be enrolled and billed for the CTS Student Health Plan (for Group coverage). A student or vicar may not insure himself through an individually underwritten plan. Waiver requests will be considered only for students or vicars who have valid coverage through the Military or VA Plan, or a group plan through one’s spouse who is employed, or through a parent’s group plan. Also please note that final-year vicars are eligible for coverage through Concordia Plan Services (see below). Waiver Petition Forms will be available on-line after August 15, 2008. The fall waiver petition deadline is September 15. Go to www.uhcsr.com, click on “College Students”, then click on “View Policy Details”, then click on “Select Your School” and go to Concordia Theological Seminary, Fort Wayne.

All vicarage congregations are encouraged to appropriately pay the premiums for their vicars directly to CTS, to be credited to the student’s account (see Insurance Premiums and Payments). The vicarage congregation should send a check payable to CTS each term (fall, winter and spring – see brochure for dates) so that his account is appropriately paid.

Although the vicarage congregation may wish to enroll the spouse and children in a separate, individually underwritten plan, the vicar may not be enrolled with his family in this type of policy (see requirement for vicars above).

The Concordia Plan Services will not accept third-year vicars, but final-year vicars are eligible for CPS coverage if the vicarage congregation desires to provide it. If a congregation is part of another health insurance plan, please check with the administrator of that plan regarding coverage.

The Vicarage Congregation’s Obligation:
Again, the vicarage congregation’s only responsibility toward these premium costs, as mandated by Synod’s Council of Presidents, is a maximum of $6,000 per year. However, some congregations recognize the necessity of health insurance for all family members and contribute more than this toward the vicar’s family’s health insurance costs. In the event that the cost of the vicar's health insurance is less than $6,000, the vicar is not entitled to the difference.

Insurance Premiums and Payments:
Vicars: A congregation’s contribution toward a vicar’s health insurance should be made directly by check to CTS, payable to the student’s account, or by check to any other company providing coverage, so that the vicar/student does not pay income tax upon the contribution toward his health insurance.

Dependents: A congregation’s contribution toward dependent coverage, for the vicar’s family, should be made directly by check to The Harbour Group at the address listed above (if using the Seminary’s dependent plan), or to another provider (if using alternate dependent coverage).
