Below are links to the various evaluation reports supervisors and vicars are required to complete. The Microsoft Word documents can be downloaded and saved to your personal computer for you to type out the report and return via email to the Vicarage office. If you have trouble with the Word documents or do not have a software program capable of reading and writing Word documents, you may also download the Adobe PDF version to print and complete manually, returning via U.S. Mail.

Report Deadlines (Supervisors and Vicars)

• If you are an M.Div Vicar, please complete and email your evaluations during the 5th and 11th month of vicarage.
• If you are an A.R. or M.Div. delayed Vicar, please complete and email your evaluations during the 3rd and 6th month of vicarage.
