Concordia Theological Seminary, Fort Wayne, offers health coverage through Concordia Health Plan (CHP). Concordia Plan Services and its Board of Directors understand that many seminary students have substantial financial constraints and they have worked diligently to make student coverage comprehensive and competitive.

The Concordia Seminary Student Option (Option CSS) through CHP provides comprehensive coverage to meet the needs of students and their families. The Seminary Student option has a $500 deductible for individuals, and a $1,000 deductible for families. Once the deductible has been satisfied, a member pays 20% coinsurance on network expenses. Option CSS covers medical, prescription drug, mental health and substance abuse expenses, as well as additional features.

As a PPO option, Option CSS offers students greater benefits if they access care through a network physician and use network providers for health services.

All residential seminary students enrolled in at least seven credit hours, international students, as well as those participating in a foreign exchange program, internship or vicarage are required to enroll in the Concordia Health Plan unless they qualify as “eligible opt-outs” according to CHP guidelines. Eligible reasons for opting out include coverage under spouse’s or parent’s group health plan, coverage as dependent under a spouse who is also enrolled in CHP as a worker, covered under a military plan as a retiree and several others. The full list of eligible reasons for opting out is given on the enrollment form. Eligible opt-outs must still return the form stating that they are declining health coverage due to one of the eligible reasons selected on page 3 of the Enrollment Form.
